Thank you for your interest in the Minnesota Credit Union Employee Benefits Plan (Plan). Below you will find information regarding the plan options being offered and instructions on how to receive an insurance quote.

The Quote Process
To request a medical quote from Medica:
This entire process should take between 5 to 7 business days, assuming your initial submission is complete.

Eligibility

- Head-Quartered in Minnesota; and
- A Member in good standing of the Minnesota Credit Union Network (MnCUN).
To be eligible, a CUSO must be:
- Owned, at least in part, by a MnCUN Member credit union; and
- Head-Quartered in Minnesota.

The Quote Process
To request a medical quote from Medica:
- Submit the information outlined below to Capstone Administrators, our third-party administrator.
- Capstone will review your submission to make sure it is complete.
- If the submission is complete, Capstone will forward it to Medica to prepare a quote for the insurance premium.
- If any information is missing, Capstone will contact you directly with instructions.
- After Medica has completed preparing your insurance quote, they will send it back to Capstone to add the applicable administrative fees.
- After Medica has completed preparing your quote, they will release it to Lockton Companies, our plan broker.
- Jeff Anderson or Amy Kuester from Lockton will then contact you to set up a time to review your proposal.
This entire process should take between 5 to 7 business days, assuming your initial submission is complete.

Preparing Your Request for Medical Quote
Gather Information and Complete the Forms: Which list of information and forms needed to submit depends upon the number of enrolled, and in some cases eligible employees* at your organization. Review the lists below and determine which one applies to your organization. Links to each form are included in the list. Each list included links to the required forms. Please note the quote will not be processed until all required information is received by Capstone.
*Your organization can set the eligibility requirements for employee participation in the plan, however, the minimum may be set no lower than 20 hours per week and no higher than 30 hours per week.
For Organizations with less than 5 potential enrolled employees:

WHY DO WE ASK FOR RENEWAL RATES?
Claims data and current rates only provide historical information. They do not provide insight on future claims or risks, which are important in risk rating your organization. Only your current provider has access to future claims data. The renewal rates give us some insight into what your current provider is seeing. This allows us to risk rate your organization more accurately and hopefully provides for a more competitive quote.

- Completed Census Form**
- Provide health history forms for all eligible employees
- Provide current summaries of benefits and coverage (SBCs)
- Provide current rates by plan design
- Current rates for all medical plans, including renewal rates if available.
For Organizations with 5 or more potential enrolled employees and less than 50 eligible employees:
- Completed AHP Employer Participation Form (.pdf)
- Completed Census Form**
- Provide current summaries of benefits and coverage (SBCs)
- Provide current rates by plan design
- Current rates for all medical plans, including renewal rates if available.
For Organizations with more than 50 eligible employees:
- Completed AHP Employer Participation Form (.pdf)

- Provide current summaries of benefits and coverage (SBCs)
- Most recent 12 months of claims data. Include monthly breakout with total membership and high claimant information if available
- Current fully insured rates or self-funded claim factors by plan, including renewal rates if available.

To request a quote for these coverages from Guardian, you will submit the information outlined below to Capstone Administrators.
